Hospitalization can lead to disability, particularly in older adults. Patients are often asked to follow up with their primary care physician after hospitalization but often can’t get an appointment quickly enough. With our limited patient panel size, rest assured we can always get you in to see the doctor shortly after your hospitalization to go over your hospital course, medications and care plan.
